Description
The ADR Template provides a consistent structure for recording an architecture decision: the context that led to it, the decision itself, and the trade-offs considered. It mirrors the format Project OS already uses internally to track its own architecture decisions.
Purpose
To make sure architecture decisions are written down with their reasoning intact, rather than living only in a person's memory or a chat thread.
Usage
Teams copy the template at the start of a new architecture decision and fill in each section before the decision is considered final. The template itself is still evolving as more teams adopt it.
